Contact Lens Evaluation

The Vision Care Customer Service Act governs how contact lenses are prescribed in the state of Washington. Shortly you will see a practitioner who can perform an evaluation for contact lenses. These tests are not part of your routine vision exam, and most insurances do not cover the cost of the evaluation. Contact lens fees are non-refundable and do not include the cost of contact lens materials. Even if there is no change to your prescription, to obtain an updated valid prescription to order contact lenses this evaluation must be performed. I acknowledge that I have received a copy of my glasses prescription as well as my contact lens prescription post exam. I can contact the office for a copy of either prescription should I lose the physical copy I received today.

Fee Range:

• Evaluation for New Wearers: $130-$195
• Evaluation for Current Wearers: $75-$160
